Annual Health and Medical Record - BSA Form
Sick of having to stop down at Council for a medical form or dig through messy files at home? Thankfully, that form can now be found online:
Click here for the official BSA Health Form.
As always, our Scouts are required to have up-to-date medical records, so double check to ensure all info is current and accurate.
Once filed, the form is valid for 12 months.
